Minnesota residents 18 years of age and older with specific medical conditions may be eligible for the use of medical cannabis.
Are you interested in becoming a medical cannabis patient?
- You must be a resident of Minnesota and 18 years of age or older
- Medical cannabis is not recommended for women who are pregnant or breast-feeding or individuals with a personal or family history of psychosis
- You must have one of the following conditions to qualify:
Post-traumatic stress disorder (PTSD)
Chronic pain
Cancer (If your illness or its treatment produces one or more of the following: severe or chronic pain; nausea or severe vomiting; or cachexia or severe wasting)
Glaucoma
HIV/AIDS
Tourette syndrome
Amyotrophic lateral sclerosis (ALS)
Inflammatory bowel disease, including Crohn’s disease
Seizures, including those characteristic of epilepsy
Severe and persistent muscle spasm, including those characteristic of multiple sclerosis (MS)
Terminal illness with a probable life expectancy of less than one year · Intractable pain
Autism spectrum disorder
Obstructive sleep apnea (OSA)
Alzheimer's disease
Sickle cell disease
Chronic motor or vocal tic disorder
How do I get certified?
- Please bring a completed and signed copy of the Patient E-mail and Acknowledgement Form to your appointment. We suggest not using an email address that others can access, or one that belongs to your employer. If you do not have an email address, please contact MDH at 651-201-5598 and they will provide you with one.
- Please bring all pertinent health care records pertaining to your qualifying condition with to your initial consultation. If you do not have a copy of them, we can have you sign a release of information (ROI) electronically prior to your visit so that your provider can review the records during your visit. Please note: our provider cannot certify you for medical cannabis without reviewing pertinent medical records pertaining to your qualifying diagnosis.
- If you meet the criteria for medical cannabis in Minnesota and have the appropriate records and forms at your initial consultation, our provider can certify you (if deemed appropriate) at your first visit. We recommend a follow up visit after you start using medical cannabis to review its efficacy and any potential side effects. Minnesota Department of Health requires an annual visit for re-enrollment.
What happens after my appointment?
- Your provider will print out an appointment summary and a list of medications that you are currently taking. Bring this document with you when you go to pick up your medical cannabis.
- Once your provider certifies you with the Office of Medical Cannabis (OMC), you will receive an email from the OMC confirming your condition and providing you with a unique registration link for the Medical Cannabis Patient Registry.
- Once your registration is approved, you will be notified by email.
- Find a medical cannabis dispensary close to you. Medical Cannabis Dispensary Locations
- You will need to submit a new Patient Self-Evaluation each time you go to the dispensary to request medical cannabis. You can find this form on the Medical Cannabis Patient Registry site.
